Toxicity goes up when you use potions on the NPC's and will go down over time.
When your first get Melissa she has a high level toxicity and if you use a potion on her as soon as you get her she will mutate several times but if you wait for about a week then use potions on her she is ok.
You can see what or how high there toxicity is with "Mind Read" spell.
